　　稱呼：平時(shí)怎么稱呼，這里就怎么寫，后面加冒號(hào)。這個(gè)地方往往是領(lǐng)導(dǎo)、相關(guān)負(fù)責(zé)人等。

　　正文：請(qǐng)假理由。直接寫請(qǐng)假的原因，起止時(shí)間。如果有什么事要交代，也可以在寫完請(qǐng)假原因之后接著寫要交代的事情。正文寫完后要加祝頌語(yǔ)，祝福對(duì)方，這個(gè)是所有公文里表示對(duì)對(duì)方的友好。

　　署名：寫在正文下右下方第三行處。如果是學(xué)生向老師請(qǐng)假，在名字前面加上“學(xué)生”二字。

　　日期：寫在署名下面，另起一行。也可以不寫年，最好在月日下面寫上上午或者下午，也可以寫上時(shí)分，必要時(shí)加上地點(diǎn)。

　　寫作要求：請(qǐng)假條最重要的內(nèi)容是寫明“請(qǐng)假原因和日期”，一定要寫明確，不能只寫“因有事請(qǐng)假”。
